*  How has your health, fitness level, weight, etc. changed over the years? What has influenced this?
*  Do you practice preventative health care?
*  Do you seek medical care when you first suspect a problem or wait until it’s more serious?
* Are there health-related issues which directly affect the way you live your life?
*  Do you approach your own health and well-being differently than that of your family members?
*  How do you maintain your mental well-being?
* Who do you consult when you have a health concern? Family? Friends? Internet? Doctor? Books? Nobody?


Documenting this part of your life now may help others…

* Become more aware of family health history.
* Gain inspiration from any efforts you have made to improve your own health.


Other ideas

* Create a health and wellness album or binder for each family member. Use it to track medical records, as well as layouts and other memory-related items.
* Create a health and wellness guide in the form of a scrapbook or mini album containing your advice, stories and inspiration for living the “good life.”
